Which one these two alternatives would you like to see in the Game first?
Both is gonna happen! The poll mainly decides what happens first.
New Main Quest = Like Goblina's Dungeon, The Cheese Factory, Watertown etc. A new Goblin for the collection.
Barrel Companion Quest = Small adventure, or happenings together with Barrel. Similar to what you've gotten with Anvil.
