There was simply no real pain in any previous Virtual World (aside from the short moment in ALO when they deactivated the pain absorber), injuries in Virtual worlds were at worst the feeling of a limb having fallen asleep, some tingling numbness, no real pain. The STL brings with is real pain as it would be IRL but without the bodies defense mechanisms like shock and endorphins, so you technically can feel even more pain than IRL. The anime kind of did Eugeo dirty, having Kirito show up Zink in episode 2 even thought it wasn't supposed to happen and now cutting out Eugeo getting the chance to get back at him and show him up in a duel adapting his sword skills beyond what Kirito had already taught him.
