They honestly don't depict it super well in the anime, but the scar on Kei is a very deep and gruesome scar that is likely the result of a near fatal stabbing. Related to that, there is actually a positive for Kei in the scene in the boiler room. Other than now having Kiyo's protection. Kei has a very vain outlook on beauty and believes girls bodies should be soft and beautiful so she hates having the ugly scar ruining her body. She is self conscious that the scar would ruin her image for men. However, because Kiyo sees her scar and does not have a negative reaction to it and instead accepts it as a part of her past, Kei starts to view him as different than other guys. Her outlook on the scar and her personal image also improves with Kiyo's acceptance. So although Kiyo obviously did not do it on purpose, this event ended up positively affecting her mental health in a way!
