I think it's for the best that she didn't confess or anything like that. For one thing, I'm positive Itadori wants to avoid another Junpei scenario. And for another, to put it bluntly, Itadori is doomed to die. After all the fingers are gathered and Mahito is FINALLY dead, they're gonna kill him.

There are a bunch of scenes that, to manga readers, are very obvious as to what they mean but not for anime-onlys. I think Mappa is generally good about giving something good for both manga readers and anime-onlys in OPs
