Sanguine Swarm

Weapon (Double-bladed Scimitar), very rare (requires attunement)

This double-bladed sword is commonly wielded by a particular sect within the cult of The Blood Mother. These warriors, known as the "White Blood," are those who survived the rite of passage through the maze but were unable to become Blood-Seekers due to a permanent blood curse inflicted by the Queen of Heart. This curse transformed them into bloodthirsty beings, dependent on the blood of living creatures for survival. Despite their affliction, those who remain devoted to their goddess are permitted to stay within the sacred bounds of Tishlash Mountain, serving as the outer sect responsible for guarding its outskirts.

The White Blood warriors are distinguished by their use of this cursed blade, which grants them the power to channel their blood into swarms of bat-like creatures. Ironically, the curse of the sword has a unique effect on them—it helps them control their bloodthirst, allowing them to maintain their sanity and continue their service to the cult.

Blood Bats. As a bonus action, you can reduce your current and maximum hit points by any amount of your choice (minimum of 10 hit points) to summon a swarm of bats formed from your own blood. The swarm appears in an unoccupied space within 30 feet of you. The swarm has the statistics of a Swarm of Bats, but its hit points are equal to twice the amount of hit points you sacrificed. The swarm uses your attack modifier for its attack rolls. The swarm is under your control and follows your mental commands, acting before or after your turn as you choose. While the swarm occupies your space or an ally’s space, that creature has half cover and their weapon attack deal an additional 1d4 necrotic damage.

Each time you reduce a creature with blood to 0 hit points, the swarm gains a number of hit points equal to 1d6 for a Small creature, plus an additional 1d6 for each size category larger than Small (2d6 for Medium, 3d6 for Large, etc.). When the swarm reaches 30 hit points, it becomes Large, and when it reaches 60 hit points or more, it becomes Huge.

The swarm lasts for 1 hour, until it is reduced to 0 hit points, or until you dismiss it as a bonus action. Your hit point maximum returns to normal after you complete a long rest. Once you use this feature, you cannot use it again until you finish a long rest.

Blood Coat. While within 10 feet of the swarm, you can use a reaction to expend the swarm’s hit points when you make an attack roll, a saving throw, or when you are targeted by an attack. For every 5 hit points expended, you gain a +1 bonus to your roll or your Armor Class, up to a maximum of +3. This bonus lasts for that one instance only.

Curse - Bloodthirsty. The Sanguine Swarm is cursed, and becoming attuned to it extends the curse to you. While cursed, you are unwilling to part with the scimitar, keeping it within reach at all times. The weapon's thirst for blood influences your mind, causing you to crave the lifeblood of others. After using the Blood Bats feature, you develop an intense thirst for blood. You must drink a pint of fresh blood from the same type of creature as you within 24 hours or suffer 1 level of exhaustion. This level of exhaustion cannot be removed until you satisfy the thirst.

And it got me thinking, where should I put a 'vampire' in this setting... and then I remember we have a blood cult already, the one with Hemogoblin~! So I made it related to that cult to expand the lore a bit more~

Love the item, design and the swarm mechanic, but a scimitar with a pointy bit below the handle hurts my feelings a bit

Yeah, I don't want it just to suck blood and empower the user since, technically, Integral Ichor is already doing that. So I tried to come up with different abilities that are also visually somewhat cool~ XD

Love the swarm aspect! This feel like it can add to other vampire items without interfering or being more of the same!
