I think the movie leans more towards its all fantasy but the book leans more towards he really did it. But regardless, the point is that these people are so self absorbed that none of them would notice either way. Like you said, they don't even listen to him and sometimes even get his name wrong.

Even when you watch it again and start to notice things you didn't the first time, it's still hard to what happened. It's based on a book that I think remember being a critique on American superficiality and general lack of accountability in the upper class. I want to say that's why nobody actually listens to what the main character is saying because everyone is too self-absorbed to actually cares about anybody. And nothing is ever done about the destruction he causes (if any of it is real), just like for example the damage the financial industry causes in the US in real life.

I highly recommend The Rules of Attraction, one of the characters in it is Sean Bateman who is actually the younger brother of Patrick Bateman (in the novels).
