And he possesses a set of skills that's hard to come by in the underworld. Most Doctors that operate in those circles have either lost their medical license (possibly due to malpractice or incompetence) or have some kind of alcohol or drug problem. Somebody like Tenma who's hyper-competent and very much a straight arrow would be a hot commodity.
