Actually, whether or not she could have feasibly prepared for the situation is not irrelevant when the question was simply could she have possibly known about it... Her having to clean up after the mess was not a question; I was just curious if there was something I missed that may have tipped her off, and she failed... I'm comfortably aware of her responsibilities as a leader.

She's the commanding officer. She fucked up in not being prepared for such a thing. Regardless of whether you think she could've feasibly prepared for it or known about it is irrelevant, the fact of the matter is that those men and their lives were her responsibility, hers to protect, and she failed, and as such it's up to her to put them out of her misery. You don't get to wiggle out of responsibility with the excuse of throwing up your hands and lamenting that there was nothing you could do or that you couldn't have known. Not when people's lives were the price. It's your job to know and to be prepared. If you can't accept the responsibility of the lives of the people under your command, you're not fit to be a leader of soldiers.
