The doctor is a pussy but he didn't want to be killed until his team's goal is complete, he wants to see the Demon Realm cross into the living world, if Yusuke kills him now he won't know if all of humanity will be extinct, if the tunnels open he'll at least die getting what he wants.

The doctor did not live with this deranged personality, he was at first a man who got his medical degree who only later developed psychic powers. So, even though he comes off as extremely crazy, we can wonder what happened to him that made him this way. If he went crazy instead of being crazy from the start, then a question Yusuke may have to ask himself is can this person be saved? Is death the only option? There's also the moral question that Yusuke knows God and they've said killing humans is bad, so in a sense it challenges the morality of God as to what is the right thing to do.
